Waiver & Consent Agreement *

1. Assumption of Risk. I, the undersigned parent/guardian, understand that dance auditions involve physical activity, which may result in injury. I acknowledge that my child is voluntarily participating in this audition and that I assume all risks associated with such participation. 2. Release of Liability. I hereby release and hold harmless Inspire Performing Arts Company LLC, its employees, instructors, contractors, and agents from any and all liability, claims, or demands arising out of or related to any injury, loss, or damage that may occur during the audition, whether caused by the negligence of the Company or otherwise. 3. Medical Authorization. In the event of an injury or medical emergency, I authorize the staff of Inspire Performing Arts Company LLC to seek medical treatment for my child as deemed necessary. I agree to be responsible for any medical expenses incurred. 4. Photo/Video Release. I grant permission for Inspire Performing Arts Company LLC to take photographs and videos of my child during the audition for use in promotional materials, social media, or the Company's website. I understand that my child's name or personal information will not be disclosed without further consent. 5. Agreement to Expectations. I understand that my child must adhere to the rules and guidelines set forth by Inspire Performing Arts Company LLC. Failure to comply with these rules may result in my child being asked to leave the audition. I understand that the determinations made by the coaches are final and in good faith, and I agree to be respectful in receiving and reacting to this information. 6. Parental Consent. I confirm that I am the legal parent or guardian of the above-named participant, and I have the authority to sign this waiver on their behalf. I have read and understand this waiver and voluntarily agree to its terms.
