Waiver & Consent Agreement *

By participating in any class, event, or activity offered by Inspire Performing Arts Company, I acknowledge and agree to the following: 1. Assumption of Risk & Liability Waiver I understand that participation in dance classes involves physical activity and carries inherent risks of injury. I voluntarily assume all risks associated with my participation. I release and hold harmless Inspire Performing Arts Company, its owners, instructors, staff, and affiliates from any and all claims or liability for personal injury, property damage, or other loss arising from participation in classes or related activities, whether on or off the premises. 2. Media Consent I grant Inspire Performing Arts Company permission to capture and use photographs and/or video footage of me taken during classes, events, or studio activities for promotional purposes. This includes use in marketing materials, social media, advertising, and on the company website. I understand that no compensation will be provided for the use of these images or recordings. By signing below, I confirm that I have read and understand this agreement and consent to its terms.
